TERMS AND CONDITIONS
NON-MEMBER DROP-IN SESSIONS, PERSONAL TRAINING, ALTITUDE CAMPS AND A LA CARTE SERVICES
The undersigned individual (the Member), hereby indicates their desire to become a member of Altitude Athletic Training Inc., pursuant to the terms and conditions of this gym membership agreement.
Health and Safety Attestation
All of the membership rules contained herein apply equally to members, temporary members and guests alike:
The Member attests that they are fully able to participate in an exercise regimen of their design or choosing without undue risk.
We require that every member completes a PARQ form and sign a health and wellness liability waiver.
All members have to be 16 and over to use the facility.
Studio users are advised to inform the instructor of any injuries, pains or concerns prior to using equipment.
It is necessary that all members and guests of the club wear appropriate, clean attire and foot wear when using the studio and gym.

Gym Membership Dues
Non recurring payments to be paid before training. Drop in passes and packages can be booked online or in person. Session packs are non-transferrable and non-refundable
This gym membership agreement entitles the member access to Altitude Athletic Training facilities during booked sessions so long as all dues are current.
Hours and specific location access are as follows (and subject to change):
Monday: 5:00 am – 8:00 pm
Tuesday: 5:00 am – 8:00 pm
Wednesday: 5:00 am – 8:00 pm
Thursday: 5:00 am – 8:00 pm
Friday: 5:00 am – 7:00 pm
Saturday: 8:00 am – 2:00 pm
Sunday: 8:00 am – 2:00 pm (sea-level)
*Holiday hours may apply and will be posted on our website.
COVID 19 Ammendment:
To reduce the spread of Covid-19, Altitude will be operating with TEMPORARY schedule changes. Changes back to the regular schedule will depend on Covid-19 policy developments.
Specific Entrance: One door – at the front, to be accessed during operating hours.
*All aspects of our gym are barrier free and accessible.
Personal Training and A La Carte Services
Personal Training and A La Carte services are offered by the gym. If they are not included in the member’s pass or if the member wishes to purchase more sessions, Altitude Athletic Training reserves the right to demand additional fees for the additional services that the Member chooses to participate in.
Personal training sessions are to be booked at least 24 hours in advance and up to one year into the future. No show fees for personal training sessions are 50% of the session fee without 24 hour notice.
A La Carte services are to be booked at least 24 hours in advance and up to four weeks into the future. No show fees for missed coach assessments/appointments/meetings (both virtual and in facility) are $50 without a 24 hour notice. They are waived for emergencies ONLY.
Altitude Drop-in Session and Session Package Booking Policy
Individuals who have purchased a drop-in session or session package may book sessions online, over the phone or in-person. Sessions can be booked at least 1 hour in advance of session start time and at most 7 days in advance of session start time. A minimum of 1 hour’s notice is required to cancel and/or reschedule sessions. Missing a session and failing to provide sufficient notice will result in a written warning for the first incident and a $25 no-show/late fee for all subsequent incidences. This fee is waived for emergencies ONLY.
The access pass membership serves as a liability release, pursuant to which the Member agrees to indemnify and hold Altitude Athletic Training harmless against any and all claims of loss or damage without limitation.
Gym Rules and Participation
All of the membership rules contained herein apply equally to members, temporary members and guests alike.
The Member hereby agrees to abide by all posted safety guidelines and regulations while using Altitude Athletic Training’s facilities and equipment. This includes but is not limited to changing guidelines regarding Covid-19 precautions.
Additionally, the Member agrees to dress and conduct themselves in a manner deemed appropriate for a fitness facility.
The Member shall not consume drugs, alcohol, or tobacco products on Altitude Athletic Training’s property.
The Member agrees not to photograph or videotape others without their permission on Altitude Athletic Training’s property.
Members are to use lockers for the period they are at the gym. Lockers will be cleared at the end of the day and all items not collected after a period of two weeks will be donated. Altitude Athletic Training is not responsible for lost or stolen items.
Altitude Athletic Training reserves the right to revoke the Member’s access if these terms are violated.

The access pass Membership shall be considered binding upon the Member and Altitude Athletic Training, and shall be upheld and enforced in accordance with the laws of Ontario, Canada. Any legal proceedings related to this gym membership contract shall take place in courts located in Toronto, Ontario, Canada.
The signed parties hereby enter into this contract with one another, in acknowledgement and acceptance of the terms listed above.
