TERMS AND CONDITIONS

BASECAMP, SUMMIT AND THE A TEAM

The undersigned individual (the Member), hereby indicates their desire to become a member of Altitude Athletic Training Inc., pursuant to the terms and conditions of this gym membership agreement.

Health and Safety Attestation

All of the membership rules contained herein apply equally to members, temporary members and guests alike:

The Member attests that they are fully able to participate in an exercise regimen of their design or choosing without undue risk.

We require that every member completes a PARQ form and sign a health and wellness liability waiver.

All members have to be 16 and over to participate in a membership at Altitude (both online and in facility).

Studio users are advised to inform the instructor of any injuries, pains or concerns prior to using equipment.

It is necessary that all members and guests of the club wear appropriate, clean attire and foot wear when using the studio and gym, and while participating in activities with Altitude (both online and in facility).

Gym Membership Dues

Basecamp: The Member agrees to pay in either 3 monthly instalments of $250+HST or 1 instalment of $675+HST prepaid. These dues may be paid by automatic debit or credit card transaction online, through the phone, or in-person. Memberships are non transferrable and nonrefundable. The Basecamp membership automatically renews every 3 months.*

 

Summit: The Member agrees to pay in either 3 monthly instalments of $300+HST or 1 instalment of $810+HST prepaid. These dues may be paid by automatic debit or credit card transaction online or in-person. Memberships are non transferrable and nonrefundable. The Basecamp membership automatically renews every 3 months.*

*6, 9, and 12 month discounts are available. Please contact Altitude Athletic Training for more information. Option to prepay or pay monthly is available. Prepaying for the term would provide an additional 10% discount.

A Team: A Team benefits are for clients who join before in our pre-launch period betwen June 2019 and July 2021. A one time reservation fee of $100 is to be paid at the time of registration; this will be refunded if the member chooses to opt out anytime between registration and a week after Altitude’s launch date. This fee can be waived if the member prepays for the entire 3 months, at $600+HST. The Member agrees to pay in either 3 monthly instalments of $200+HST or 1 instalment of $600+HST prepaid. The membership package for the first three months is equivalent to that of Summit. If A Team members choose to renew their membership, they get a preferred rate of $200+HST for Basecamp and $250+HST for Summit.

Member Access

This gym membership agreement entitles Basecamp and Summit members access to Altitude Athletic Training facilities (equipment bookings) so long as all dues are current. 

 

Loaned Items

Summit members receive a box with tools to get them started and help them throughout their journey. The Polar H10 Heart Rate Monitor and powerband is an important assessment tool that we loan out to our members. The Polar H10 Heart Rate Monitor must be returned within a week after the Member’s program end date and the powerband must be returned within a week after the Member’s program start date. A late penalty of $5/day will occur after 7 days. If either or both of the items are lost, the Member must notify their coach and pay for a replacement.

Hours and specific location access are as follows (and subject to change):

Regular Hours:

Monday: 6:00 am – 8:00 pm

Tuesday: 6:00 am – 8:00 pm

Wednesday: 6:00 am – 8:00 pm

Thursday: 6:00 am – 8:00 pm

Friday: 6:00 am – 8:00 pm

Saturday: 8:00 am – 12:00 pm

Sunday: 8:00 am – 12:00 pm (sea-level)

*Holiday hours may apply and will be posted on our website.

COVID 19 Amendment:

To reduce the spread of Covid-19, Altitude will be operating with TEMPORARY schedule changes. While opening hours remain the same, drop in access to the gym for Basecamp and Summit members will be on a modified schedule. Changes back to the regular schedule will depend on Covid-19 policy developments.

Specific Entrance: One door – at the front, to be accessed during operating hours.

*All aspects of our gym are barrier free and accessible.

Personal Training and A La Carte Services

Personal Training and A La Carte services are offered by the gym. If they are not included in the member’s membership dues or if the member wishes to purchase more sessions, Altitude Athletic Training reserves the right to demand additional fees for the additional services that the Member chooses to participate in.

Indemnification

The Basecamp membership serves as a liability release, pursuant to which the Member agrees to indemnify and hold Altitude Athletic Training harmless against any and all claims of loss or damage without limitation.

Gym Rules and Participation

All of the membership rules contained herein apply equally to members, temporary members and guests alike.

The Member hereby agrees to abide by all posted safety guidelines and regulations while using Altitude Athletic Training’s facilities and equipment. This includes but is not limited to changing guidelines regarding Covid-19 precautions. This includes but is not limited to: face masks must be worn while in the facility; maintaining proper social distancing measures outlined; using wipes to wipe down all equipment the Member has touched.

Additionally, the Member agrees to dress and conduct themselves in a manner deemed appropriate for a fitness facility.

The Member shall not consume drugs, alcohol, or tobacco products on Altitude Athletic Training’s property.

The Member agrees not to photograph or videotape others without their permission on Altitude Athletic Training’s property.

Members are to use lockers for the period they are at the gym. Lockers will be cleared at the end of the day and all items not collected after a period of two weeks will be donated. Altitude Athletic Training is not responsible for lost or stolen items.

Altitude Athletic Training reserves the right to revoke the Member’s access if these terms are violated.

Altitude Equipment Booking Waitlist Policy:

Basecamp and Summit

Member bookings are done in the Mywellness App. Members book the equipment/spots they want to use in advance. Spots are booked in 30 minute increments. Bookings start 5 days before the session begins at 12:00 am and close when the session begins. Members are allowed a max number of 4 active bookings at any one time, totalling 120 minutes allowable booking time.

Equipment/spots will have waitlists when they are full. If you are on a waitlist for a piece of equipment beginning at noon or later and a spot opens up, you will be automatically enrolled in the waitlist up to 1 hour before the class start time. For morning bookings (before noon), automatic enrolment will end at 9pm the night before.

Bookings close when the session begins and cancellations close 12 hours before the session begins. You are still welcome to book a piece of equipment right before the start time, provided there are available spots. If you did not sign up in advance, you may drop by the gym before the start time and join if there is an available spot.

If you reserved your spot and are a no-show, you get one written warning. After that, you may be charged a late/no-show fee (of up to $25). You can give up your reserved spot without a fee as long as it is 12+ hours before the class start time. You may sign up for a class online 48 hours before the start time.

Due to Covid-19, please ensure that you book your equipment ahead of time so that we can ensure that we are properly staffed.

No show fees for missed coach appointments/meetings (both virtual and in facility) are $100 without a 48 hour notice. They are waived for emergencies ONLY. No show fees for personal training sessions are 50% of the session fee.

 

 Gym Membership Cancellation:

Basecamp Membership may be cancelled by providing written, online or in-person notice at Altitude Athletic Training. If the contract is cancelled prior to the contract expiration date, the Member agrees to pay a cancellation fee equivalent of 50% of the remaining membership.

If the Member must cancel this gym membership contract due to medical reasons, the Member must demonstrate a doctor’s note and the cancellation fee will not apply.

Summit Membership may be cancelled by providing written, online or in-person notice at Altitude Athletic Training.

There is no cancellation fee if the Member provides 21 days notice. Cancelling memberships less than 21 days in advance will result in a $100 fee, tax free, and payable by automatic debit or credit card transaction online, through the phone, or in person due on receipt.

Gym Membership Holds:

Basecamp and Summit members can place their membership on hold for a period of one month. There is a hold fee of $30, tax free.

Binding Law

The Basecamp Membership shall be considered binding upon the Member and Altitude Athletic Training, and shall be upheld and enforced in accordance with the laws of Ontario, Canada. Any legal proceedings related to this gym membership contract shall take place in courts located in Toronto, Ontario, Canada.

Acceptance

The signed parties hereby enter into this contract with one another, in acknowledgement and acceptance of the terms listed above.
