TERMS AND CONDITIONS
The undersigned individual (the Participant) hereby indicates their desire to participate in activities operated by Altitude Athletic Training Inc., pursuant to the terms and conditions of this agreement.
Health and Safety Attestation:
All of the membership rules contained herein apply equally to members, temporary members and guests alike:
- The Participant attests that they are fully able to participate in an exercise regimen of their design or choosing without undue risk.
- The Participant agrees to complete a PARQ form and sign a health and wellness liability waiver prior to training.
- Participants must be 16 and over to use the Altitude Athletic Training facility unattended without the supervision of an adult.
- Participants are advised to inform Altitude Athletic Training staff and coaches of any injuries, pains or concerns prior to using equipment and engaging in training.
- It is necessary that all participants, members and guests of Altitude Athletic Training wear appropriate, clean attire and foot wear when using the amenities and gym, and while participating in activities operated by Altitude Athletic Training.
Gym Rules and Participation:
All of the gym rules contained herein apply equally to participants, members, temporary members and guests alike:
- The Participant hereby agrees to abide by all posted safety guidelines and regulations while using Altitude Athletic Training’s facilities and equipment. This includes – but is not limited to – changing guidelines regarding Covid-19 precautions.
- The Participant agrees to dress and conduct themselves in a manner deemed appropriate for a fitness facility.
- The Participant shall not consume drugs, alcohol, or tobacco products on Altitude Athletic Training’s property.
- The Participant agrees not to photograph or videotape others without their permission on Altitude Athletic Training’s property.
- Participants are to use lockers for the period they are at the gym. Lockers will be cleared at the end of the day and all items not collected after a period of two weeks will be donated. Altitude Athletic Training is not responsible for lost or stolen items.
Altitude Athletic Training reserves the right to revoke the Participant’s access if these terms are violated.
MEMBERSHIP PLAN RENEWALS, CHANGES AND CANCELLATIONS:
Membership fees are subject to economic conditions, inflation and annual reviews of pricing structures and therefore can be changed at any time; written notification will be provided in the case of price increases to services.
MEMBERSHIP DUES AND AUTOMATIC RENEWAL POLICY:
Monthly memberships will automatically renew each month at the same payment terms and billing date established at time of enrollment. All prepaid plans (3, 6, and 12 month blocks) are paid upfront and will automatically renew upon expiration at the same rate and terms as the initial membership. Any changes to the plan, or cancellation must be received in advance of the next billing cycle. It is the responsibility of the Participant to monitor membership expiration and renewal dates.
Basecamp: the Participant agrees to pay the membership dues below according to his/her/their selected term length:

Membership dues may be paid by automatic debit or credit card transaction online, by phone, or in-person. Memberships are nontransferrable and nonrefundable. The Basecamp membership will renew automatically at the same rate and terms as the initial membership.
Summit: the Participant agrees to pay the membership dues outlined below according to his/her/their selected term length. On January 5th, 2023, the Summit membership and all discounted variations (family discount and 60 Colborne discount) will no longer be offered at the membership rates outlined above. On January 5th, 2023, the membership rates will be as follows:

Summit Inaugural Period Pricing End Date and Price Changes on January 5th, 2023
The membership dues below are valid from the inaugural period between July 16th, 2021 until January 5th, 2023 and are subject to change on January 5th, 2023.

Any changes in the membership dues on January 5th, 2023 will not apply to current active members or members on a designated hold period who have not submitted a cancellation request in-person, in-writing or over the phone to Altitude Athletic Training (“Inaugural Summit Members”). New clients purchasing a Summit membership at any time prior to January 5th, 2023 will be on inaugural membership rates and classified as Inaugural Summit Members, regardless of their effective membership start date, so long as minimum of one month’s payment is made prior to January 5th, 2023.
Inaugural Summit Members will continue at inaugural pricing so long as his/her/their membership remains active or on an allocated hold period, all dues remain current and no cancellation request is made. Inaugural Summit Members can renew at inaugural pricing for any of the month-to-month, 3-month, 6-month or 12-month term lengths assuming above is true, regardless of their prior term length.
Please note changes may apply to the membership dues of Inaugural Summit Members in the future. Inaugural Summit Members will receive advance notice if and when changes to their membership dues outlined above are scheduled to apply.
Changes in the membership dues will apply to the following:
- New clients purchasing a Summit membership for the first time after January 5th, 2023
- Inactive clients who submitted a cancellation request at any time in-person, in-writing or over the phone to Altitude Athletic Training
- Clients who’s membership dues are not up-to-date
- Clients who have cancelled Summit memberships in-person, in-writing or over the phone to Altitude Athletic Training initially purchased prior to January 5th, 2023 and are re-joining on a Summit membership after January 5th, 2023
- Active members (or those on a designated hold period) on memberships other than the Summit membership, including the Basecamp membership, choosing to transfer to the Summit membership after January 5th, 2023.
- Active clients on personal training packages or any other service choosing to transfer to the Summit membership after January 5th, 2023.
Summit+: the Participant agrees to pay the membership dues below according to his/her/their selected term length:

Summit+ Weekly Personal Training Sessions
The Summit+ membership includes one (1) weekly 60-minute Personal Training session with your coach for the duration of your membership term. Weekly Personal Training sessions shall renew and continue alongside Summit+ membership renewals.
Session scheduling can be done directly with your coach or at the front desk by phone, email or in-person so long as all membership dues are current and paid in full.
Summit+ members are encouraged to provide at least 24 hours notice prior to cancelling/rescheduling a session.
Personal Training sessions must be used within the membership term length period. Clients shall use one (1) session per week of his/her/their membership so long as all membership dues are current and paid in full and the membership is active. Weekly sessions not used within the membership term will be lost without any refund and shall not be carried over past the membership renewal or expiry date. Any such sessions not used within the week will be lost without any refund and shall not be carried over into the following weeks. It is the responsibility of the member to schedule weekly sessions and ensure sessions are rescheduled within that same week if necessary. While missed sessions may be rescheduled within the same week, they cannot be carried over to following weeks. I.e if you miss a session on Tuesday this week, you have until the end of the week to reschedule your session, but you wouldn’t be able to ‘double up’ the following week.
If your coach is more than 10 minutes late for your session you will be entitled to a complimentary session, along with any remaining time you still have for that session. If you are late for your session, your session will be reduced in accordance with that time. If you are more than 25 minutes late, your coach may choose to cancel the session and you may lose that session.
The A Team (Available Pre-launch Only):
The A Team benefits are for clients who join before in our pre-launch period between June 2019 and July 2021. A one-time reservation fee of $100 is to be paid at the time of registration; this will be refunded if the member chooses to opt out anytime between registration and one week after Altitude’s launch date. This fee can be waived if the member prepays for the entire 3 months, at $600+HST. The Member agrees to pay a monthly membership fee of $200+HST per month on a month-to-month agreement. Membership dues may be paid by automatic debit or credit card transaction online, through the phone, or in-person. Memberships are nontransferrable and nonrefundable. The A Team membership automatically renews every month.
The Participant agrees to the following membership cancellation policy which applies for the Basecamp, Summit and Summit+ memberships:
Month-to-Month plans and memberships can be canceled at any time, however all cancellation requests must be submitted in-person, by phone or by sending written notification via email to firstname.lastname@example.org. Please do not send cancellation messages to our Facebook page or other social media sites. No cancellation fees or penalities apply at this time.
All cancellation requests must be submitted and received prior to the Participant’s renewal date.
Please note that Summit and Summit+ members will be following a carefully designed program created by their coach. Altitude coaches spend time and resources developing these custom programs and will prepare future phases in weekly-monthly blocks of a member’s program according to a member’s goals and timeline to ensure the greatest chance at success. Altitude Athletic Training kindly asks Altitude members to respect the time and efforts of their coaches and provide advanced notice before cancellation.
LATE CANCELLATION POLICY:
For prepaid plans, cancellation requests submitted after the renewal date will be considered late cancellations and fall under the Altitude Athletic Training late cancellation policy. In a late cancellation circumstance, the Partipant will be refunded 50% of the prorated amount on the remaining membership term. It is the Participant’s responsibility to provide written notice in advance of their renewal date.
MEMBERSHIP HOLDS AND ACCOUNT FREEZES:
The Participant agrees to the following membership hold policy which applies for the Basecamp, Summit and Summit+ memberships per membership cycle. Members may put their membership on freeze or hold. The hold period varies by membership term length. The allocated hold periods for the listed membership term lengths are indicated below:

All payments drawn prior to the requested hold will not be refunded. All clients will have memberships extended by the number of frozen weeks or months.
MEMBERSHIP LAPSES AND MEMBERSHIP REACTIVATION:
If the Participant cancels their membership at any time and returns at a later date they will be subject to any rate increases at the time of enrollment.
Altitude Athletic Training will not provide refunds retroactively for any cancellation requests. Membership cancellations are processed in-person, by phone or by sending written notification via email to email@example.com. Please do not send cancellation messages to our Facebook page or other social media sites.
Altitude Athletic Training does not issue refunds if you do not use your membership.
DISCOUNTED MEMBERSHIP PLANS AND PACKAGES:
Altitude Athletic Training provides discounted rates to current residents of the 60 Colborne condo building (60 Colborne Street, Toronto, Ontario, M5E 0B7) and families who sign up together (minimum 2 family members). Please inquire with our staff for more details on discounted plans and packages.

PERSONAL TRAINING AND A LA CARTE SERVICES:
Personal training sessions and some services including select performance assessments and private room bookings are not included in Altitude Athletic Training memberships. Altitude Athletic Training reserves the right to demand additional fees for the additional services that the Member chooses to participate in.
ASSESSMENTS AND RE-ASSESSMENTS
Summit and Summit+ memberships start with one (1) two-part performance assessment.
Re-assessments are done after 3 consecutive months of training.
For month-to-month memberships, reassessments will only be done after the third consecutive month of training (three membership renewals)
For 3-month memberships, the reassessment will be done at the end of the membership term.
For 6-month memberships, reassessments will be done 3-months into the membership and at the end of the membership term.
For 12-month memberships, reassessments will be done 3-months into the membership, 6-months into the membership, and at the end of the membership term.
The purchase of a new membership term length after 1 or 2 months of training (i.e if a member where to switch to 6-months from month-to-month after 1 month of training on month-to-month) does not give the member access to another two-part performance assessment or a reassessment. The reassessment for this member will only be done have three consecutive months of training.
Altitude Equipment Booking Policy:
Member bookings are done in the Mywellness App. Members book the equipment/spots they want to use in advance. Spots are booked in 30 minute increments. Bookings start 5 days before the session begins at 12:00 am and close when the session begins. Members are allowed a max number of 4 active bookings at any one time, totalling 120 minutes allowable booking time.
Equipment/spots will have waitlists when they are full. If you are on a waitlist for a piece of equipment beginning at noon or later and a spot opens up, you will be automatically enrolled in the waitlist up to 1 hour before the class start time. For morning bookings (before noon), automatic enrolment will end at 9pm the night before.
Currently, members may cancel booked sessions right up until the session start time. Members are still welcome to book a piece of equipment right before the start time, provided there are available spots. If a Member does not sign up in advance, they may drop by the gym before the start time and join if there is an available spot.
Due to Covid-19, please ensure that you book your equipment ahead of time so that we can ensure that we are properly staffed.
A LA CARTE SERVICES (Drop-in Sessions, Personal Training Sessions, Class Packages Etc.)
Nonrecurring payments for A La Carte services and packages are to due prior to the service start date. A la Carte services and packages an be booked online, over the phone or in-person. A La Carte services and packages are nontransferrable and nonrefundable.

PERSONAL TRAINING SESSIONS AND PACKAGES
Please note that the terms below apply only to personal training clients who have purchased an A La Carte personal training package and do not apply to clients on the Summit+ membership with included weekly personal training sessions.
Session Bookings and Cancellations:
Session bookings can be made with your personal trainer or at the front desk by phone, email or in-person. All payments must be made in full, either in-person at the front desk or online prior to any sessions being undertaken or booked.
Personal training packages can be purchased in 1-month, 2-month and 3-month blocks at a set number of sessions per week. Although recommended, the monthly session sets do not need to be completed within a given month specifically. The blocks simply serve as guidelines to encourage consistency and compliance. If sessions are missed or cancelled, they will carry over and can be rescheduled to another time that is convenient to both the client and trainer.
All personal training package plans and associated session sets expiry at double the length of the block purchased, after which time any unused sessions will be lost and no refund will be made. The expiry schedule is as follows:
- 1-month PT session block: Sessions expire 2 months from purchase date
- 2-month PT session block: Sessions expire 4 months from purchase date
- 3-month PT session block: Sessions expire 6 months from purchase date
Personal training clients may extend Personal Training packs beyond the expiry day for between one month and 12 full calendar months for the following reasons only:
– Serious illness
– Serious injury
To extend your Personal Training packs you must contact the front desk.
No personal training refunds will be issued for any reason, including but not limited to relocation, illness, and unused sessions.
Personal training clients are encouraged to provide at least 24 hours notice prior to cancelling/rescheduling a session.
- If your Personal Trainer is more than 10 minutes late for your session you will be entitled to a complimentary session, along with any remaining time you still have for that session.
- If you are late for your session, your session will be reduced in accordance with that time. If you are more than 25 minutes late, your Personal Trainer may choose to cancel the session and you may lose that session, in which case no refund will be made.
- If for any reason you are unhappy with your Personal Trainer, you may transfer to another personal trainer at any given time.
- In all circumstances, any sessions used with the previous Personal Trainer remain used.
- Your Personal Trainer cannot prescribe treatment or diagnose medical conditions. They may at any time ask to consult with your doctor should any medical condition arise.
DROP-IN SESSIONS AND PACKAGES
All drop-in sessions and session packages will expiry as follows:

SAFETY, ACCESSIBILITY AND HOURS OF OPERATION
This agreement entitles the Participant access to Altitude Athletic Training facilities and equipment bookings so long as all dues are current.
Altitude Athletic Training is located on the ground level of the 60 Colborne Condo building. Therefore, we must respect the privacy and space of condo residents. All participants must use the front exterior entrance of the facility during operating hours. Altitude Athletic Training has an emergency exit which is to be used by participants in emergencies only.
All aspects of the Altitude Athletic Training facility are barrier-free and accessible. Please contact an Altitude Athletic Training staff member should you require specific accommodations.
Hours and specific location access are as follows (and subject to change*):
- Monday: 6:00 am – 9:00 pm
- Tuesday: 6:00 am – 9:00 pm
- Wednesday: 6:00 am – 9:00 pm
- Thursday: 6:00 am – 9:00 pm
- Friday: 6:00 am – 9:00 pm
- Saturday: 7:00 am – 1:00 pm
- Sunday: 7:00 am – 1:00 pm (sea-level)
*Holiday hours may apply and will be posted on social media and emailed to all members in advance
**COVID-19 Amendment: To reduce the spread of Covid-19, Altitude will be operating on the modified schedule posted above. Members will be notified of any changes to the above schedule via email in advance.
The Basecamp, Summit and Summit+ memberships and A La Carte Services (Drop-in Packages, Personal Training Packages, Class Packages, etc.) serve as a liability release, pursuant to which the Participant agrees to indemnify and hold Altitude Athletic Training harmless against any and all claims of loss or damage without limitation.
The Basecamp, Summit and Summit+ memberships shall be considered binding upon the Member and Altitude Athletic Training, and shall be upheld and enforced in accordance with the laws of Ontario, Canada. Any legal proceedings related to this gym membership contract shall take place in courts located in Toronto, Ontario, Canada.
The signed parties hereby enter into this contract with one another, in acknowledgement and acceptance of the terms listed above.
